Your parents/guardian …

·         Must have parent complete and sign the required 4 page PIAA Comprehensive Initial Pre-Participation Physical Evaluation forms which can be found on our website or in the nurse’s office or athletic office and signatures required by parents. 

·         Must give a detailed explanation if you answer “yes” to any of the questions and state whether or not the problem still exists.

You…

·          Need to bring your glasses or wear contacts.

·         Need to have an updated Tdap (tetanus) shot

ALL PHYSICAL S ARE HELD IN THE NURSE’S OFFICE AT THE HIGH SCHOOL.
